All-on-4 replaces an entire arch of teeth with a fixed bridge secured to just four implants. The two rear implants are placed at an angle rather than straight down, which lets them anchor into the denser bone toward the front of the jaw and avoid the sinus cavity above and the nerve canal below. That angling is the whole point of the technique: it makes full-arch treatment possible for many patients who would otherwise need extensive bone grafting first.
The bridge is screwed in and stays in. You don't remove it, it doesn't cover the palate on upper arches, and chewing force is far closer to natural teeth than any removable option delivers. Most patients receive a fixed temporary bridge on the day of surgery and the final bridge after four to six months of healing.

All-on-4 is often presented as avoiding grafting entirely, and for many patients it does. But severe resorption in the front of the jaw, where the angled implants need to anchor, can still rule it out. That's an imaging question, not a marketing one.
